Readmission Prevention Program
A nosocomial infection is an infection that is acquired while in the hospital. Statistics prove that after day 7 of a hospitalization, the chances of one acquiring a nosocomial infection increase significantly which will prolong their discharge.
The readmission prevention program requires our Certified Nurse Assistants to continuously observe for signs and symptoms that may signal a decline in your loved one's health status that would lead to rehospitalization. We will notify you and your loved one's physician of the changes and provide transportation to a doctor's appointment if necessary. We are proactive when it comes to identifying these changes and are effective with our communication.
